Provides support for the educational needs of pediatric patients at Helen DeVos Children's Hospital. This support includes, but is not limited to, collaborative educational planning with school systems during periods of hospitalization and to meet post-discharge needs, assisting with individualized educational plans, and implementation of the hospital-based school program.

Essential Functions

  • Identifies, in collaboration with care team and patient’s family, pediatric patients in need of educational support due to acute or chronic illness or condition. Acts as a patient / family advocate.
  • Facilitates meetings, works with the media and develops public awareness activities.
  • Collaborates with the Child Life team to provide programming for pediatric patients and families which addresses the psychosocial and developmental needs of children and teens coping with illness and injury.
  • Obtains consent from family and contacts patient’s local school for child’s current school work and assists in the flow of assignments between the hospital and school. Plans and conducts daily lessons to support patient’s schools goals and assignments. Evaluates and reports student progress to parents and school. Recommends and evaluates the need for formal testing of students if needed. Contacts local school district for continued homebound teaching services if needed.
  • Helps local school teacher to understand any new educational needs the child may have ensuring the successful return of the student to the classroom.
  • Educates local school districts on school program available at Helen DeVos Children’s Hospital.
  • Assists families in understanding their child’s educational rights as they relate to state and federal regulations or guidelines.
  • Establishes a volunteer program in conjunction with volunteer services to provide tutors to children and teens to assist in meeting the educational needs of children/teens served by the school program.

Job Description

Clinical Service
  • Provides daily instruction to patients with behavioral and emotional disorders.
  • Creates daily lesson plans and delegation of work for staff, students and volunteers.
  • Provides a safe, educational environment for the children (including classroom management

Mott Children’s Hospital within our hospital classroom and in patient rooms.  This teacher, the Fellow, will engage with patients who are significantly impacted by chronic illness and their families.  The age range of the potential patient population is Kindergarten through college with an emphasis on reading intervention and the K-8 population. GENERAL SUMMARY The School Intervention Program Specialist is an employee at Children’s Hospital of Wisconsin (CHW) whose role is to support the School Intervention Program (SIP) that supports the intersection between a child’s health and educational outcomes. This role serves as program Specialist with a primary focus on providing care coordination services for patients whose schooling has been affected by chronic and complex illnesses. This role performs an in-depth school needs assessment through structured parent and teacher interviews, analyzes key medical and school records (treatment and neurodevelopmental histories, report cards, attendance records, health and education plans), and consults with physicians and other clinical providers. Based on the responses from the comprehensive assessment process, the Specialist applies knowledge of the neurodevelopmental needs and risk factors of school-age cardiac patients to develop individualized school healthcare guidelines and a comprehensive academic recommendation report for each patient. This role is expected to attend school meetings in-person to describe the child’s diagnoses to school staff in layman’s terms and explain how their cardiac physiology may result in differences in neurodevelopment, requiring modifications and accommodations that school staff had previously not identified. As an outside consultant, the Specialist must be confident asking questions, making suggestion, and effectively advocating for the child during school meetings. The Specialist role does not frequently interface with patients, nor does it provide any level of direct academic instruction to patients. ESSENTIAL FUNCTIONS • Coordinates care among a multidisciplinary team of stakeholders: clinical team, patients, their families, and schools in alignment with best educational and medical practices. • Complies with HIPAA and FERPA confidentiality restrictions: acquires and confirms consent authorizations prior to exchanging medical and educational records with schools and other external healthcare providers. • Establishes and maintains collaborative working relationships with interdisciplinary team members, department managers and staff, patients and families, school personnel, other professional colleagues, and members of the public. Conducts multi-source, multi-method information analyses of patients’ school support needs. • Reviews patients’ medical record, including: diagnoses, comorbidities, surgical history, neurodevelopmental sequelae, activity restrictions, and limitations in strength, vitality, and/or alertness. Reviews patients’ school record, including: education plans (IEPs and 504s), health plans, report cards, and attendance records. Codes patient medical, educational, and intervention-related information and inputs into program database. • Accurately follows program protocols to conduct parent and school assessments. • Evaluates assessment data, medical and school records to identify gaps in school services for children with complex medical conditions. Recommends health, curriculum, instructional delivery, and other educational modifications and accommodations. Educates families about education law, special education services, and different types of educational plans. • Synthesizes hundreds of pages of medical records, school records, and assessment data into a concise recommendation report that this easily understood and applicable by schools. ESSENTIAL PROGRAMMATIC FUNCTIONS • Develops templates, tools, and protocols designed to improve program operational efficiency. • Designs, prepares, and promotes school workshops across the state of Wisconsin. • Spearheads policy change by working with Government Relations to determine opportunities for state funding, insurance billing codes, and proposing new educational laws for students with chronic and complex health needs. • Establishes and leads various parent support opportunities and resources. • Applies advanced problem-solving skills as it relates to program optimization and opportunities for improved patient / family / school support and satisfaction. • Contributes to staff hiring by participating in the review of resumes, interview, and final selection process. • Trains program staff on new tools, processes, and protocols; participates in the onboarding and training process of new employees. • Provides demonstrations and guided support to School Intervention Specialists; co-oversight of administrative staff. • Supports duties of the Program Manager and Program Coordinator in his/her absence. • Performs other duties as assigned. MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: • Bachelor’s degree required; master’s or other professional degree preferred. • Experience with project management required. • Advanced computer skills required: Microsoft Excel, Word, Outlook, PowerPoint, Adobe, EPIC, REDCap, internet, scheduling, and database construction and maintenance. • Database design, management, and analysis experience preferred. • Interdisciplinary and cross-functional experience in areas of education, healthcare, or behavioral health preferred. • Team leadership; experience designing/hosting workshops/trainings preferred. • Exceptional organizational, planning, analytical, problem-solving, and cross-functional skills. • Ability to work within deadlines on a consistent basis. • Superior oral, written, and interpersonal communication skills; presents conclusions and recommendations clearly and logically.   • Functions as a member of and integrates the educational programs into the interdisciplinary team to promote optimal patient care.
  • Serves as a content expert, representing the school services perspective on hospital-wide committees.
  • Develops complete, current and relevant policies and procedures in accordance with organizational objectives, departmental & program scope of service, Family-Centered Care practices, and regulatory agency requirements.
  • Integrates trends in healthcare and the profession to identify future needs for program development, space, and financial resources.
Quality/Process Improvement:
  • Ensures services meet professional standards, and regulatory and legal requirements.
  • Provides for ongoing monitoring of key indicators and utilizes the established performance improvement process to improve services/care.
  • Oversees and maintains program data collection.
  • Leads the development, oversight, and completion of department goals.
Fiscal Management:
  • Is a strong financial steward with solid understanding of all financial components of the Hospital School Program billing process (e.g. guarantee of payments, invoices, collections, etc.).
  • Develops and adheres to operating and donor funded budgets.
  • Speaks to the financial needs and philanthropic benefits with existing and potential donors.

Job Summary The Education Coordinator is responsible for the coordination, planning, and delivery of academic educational support services to patients followed by an identified service line (e.g. Dialysis, Rehab, Oncology, etc.). Educational services and coordination involves close collaboration with the healthcare team, inclusive of families and patients’ community schools, to insure effective communication of patient educational history and/or newly identified needs. This includes, but is not limited to: providing families with school letters for 504 service plans, communication with the patient’s school around curriculum planning and 504 service plans, and providing information on the impact of medical diagnosis or treatment on learning, and interdisciplinary collaboration.  School re-entry services include informing families around education advocacy issues and provision of information to schools concerning disabilities or diagnoses, and their implications on school services and/or re-entry. Job Responsibilities Evaluate patient’s psychoeducational needs (based on parent report, school report, review of available assessments, and medical treatment plan)
  • In collaboration with the school team will determine appropriateness of homebound instruction, 504 service plan, IEP and/or other school-related services
  • Discusses with patient’s caregiver any questions/concerns regarding school re-entry
  • Gathers pertinent school history and records and shares this with the healthcare team
  • Collaborates with school in the identification of tasks to facilitate re-entry (i.e. Homebound Referral Form, parent letter requesting Multi-disciplinary Team Evaluation)
  • Facilitates school re-entry by scheduling and coordinating school meetings and/or conference calls as needed
  • Documents in the patient’s medical record pertinent information gathered from the school and summarizes school re-entry plan as appropriate
Patient and Family Education Advocate/Liaison
  • Acquires parent authorization to communicate with school staff
  • Provides information pertaining to education systems and process to support family’s efforts in advocating for their child
  • Attends school meetings as appropriate, including IEP meetings
  • Conducts school visits to educate school staff on the educational needs of the child, as needed
  • Collaborates with the school and family when a need to develop a behavior management plan is identified as appropriate
  • Informs school personnel of anticipated accommodations or changes to the patient’s current education program
  • Develops contacts and network with other providers, schools, and community resources to support patients and families in meeting the educational needs of their children
Multidisciplinary Team/Communication
  • Documents educational services provided and recommendations as appropriate in the patient’s medical record
  • Incorporates support staff into the educational goals and activities of individuals or groups of patients
  • Integrates multidisciplinary team goals for the patient into the educational program and individual interactions when appropriate
  • Interprets educational program and provides multidisciplinary team with pertinent educational information by participating in psychosocial rounds, care conferences, therapeutic rounds, or discharge planning meetings as appropriate
  • Collaborates with neuropsychologists who conduct assessments to develop appropriate school-related recommendations
  • Collaborates with Hospital School Program inpatient teachers regarding any school related issues, information, and/or concerns
  • Recommends consults/referrals when student needs are beyond the scope of the school program
Required Licenses, Certifications, Registrations PA Teachers Cert - Special Ed or School Counseling Required Education and Experience Bachelor’s Degree in Special Education, Elementary Education, Secondary Education, or School Counseling.
